Clinic Information Errors
This would be any information such as - shipping address, suite number, clinic phone number. These types of errors are generally straightforward to fix. To correct them, you will need to simply email onboarding@goinfusive.com stating what information is incorrect and providing the corrected information.
Ideally this should be corrected before the forms are signed. if the documents have been signed already, it may require obtaining a new signature(s) on the document(s) with the incorrect information. This helps maintain the integrity and legality of the document.
Signature Errors
In some cases, the error may involve the signature itself. The most common mistakes we see are - (1) the documents are signed by clinical staff instead of the medical provider or (2) the provider signed it but they signed it with an email instead of their name.
If this is the case, to resolve this, we will resend a fresh form via our third-party e-signature platform, PandaDoc. This ensures that the document is properly formatted and that all signatures are in their designated areas.
